Medication Prior Authorizations 

 

If your pharmacy informs you that your prescription needs prior authorization ​please provide us with an alternate equivalent medication that is covered by your insurance whenever possible.

 

If you still require a prior authorization a $25 processing fee will apply. Please do ALL of the following:

• Have your pharmacy send the required paperwork to our office

• Call our office directly at 247-7778, ext 4 option 1 and leave a message requesing a Prior Authorization including your name, the medication, and the pharmacy. 

 

Once it has been submitted to your insurance company it may take up to two weeks for them to inform to your pharmacy of the determination.
