Referrals
If you think you need to see a specialiast, please call the office to set up an appointment to be evaluated.
You will need a face-to-face appointment before a referral can be generated for all of the following:

New medical problem
-
Sleep Studies
-
Initiating Home health care
-
All Durable medical equipment
Hospital beds
Nebulizer machines
Power wheelchairs
Commodes
Scooters
If you have already been seen and evaluated by the physician for your condition you can request your referral by calling the office at 247-7778 ext. 5. Please leave all the information about your condition, and your preferred doctor or facility.

Prescription Policy
When requesting refills please contact our office directly at 247-7778 ext. 4 to speak with a Medical assistant or leave a message and include your name, the medication, and the pharmacy information.

Every 12 months a medical history and exam is necessary for any refills.

Antibiotics and all controlled medications will not be refilled without a medical evaluation by a clinician.

Please call at least 48 hours in advance when requesting routine medication refills.

Bring in all your medication bottles for us to review for accuracy when you return for your prescription refill visit.
The physicians have decided that in order to provide the best care for their patients they do not respond to faxed refill request from the pharmacy. Instead, please call our office directly or have the pharmacy send the request through their eprescribe system.
Please allow 48 hours for all prescription refills to be addressed. Please be aware that if you are requesting a NEW prescription, or a medication change you must have an appointment.

Test Results
Please schedule an appointment with a provider so that they can thoroughly discuss your results and plan of care. Sometimes medications need to be adjusted or further testing may be ordered. To achieve the maximum benefit of your office visits it is important that you participate in your follow up care.
When scheduling for a result review or have additional questions about your results please allow at least four days for non urgent testing. You can call 247-7778 ext. 4 option 2, and leave a detailed message and a clinician will return your call within 48 hours.

Appointments
Please call our office at 247-7778 during business hours to schedule appointments Monday - Thurs 9am- 5pm and Friday 9am-noon. For your convenience we have lunch hour appointments available.
Please arrive 5 minutes early for all follow up appointments and 10 minutes early for all specialty appointments.
If your physician request a special procedure such as an Echo, Carotid doppler, or Nerve Conduction study, please call to schedule these promptly as these appointments are limited.
Because a Physician's time is valuable; same day cancellations or missed appointments are subject to a $50 fee. We ask that you give us 24 hours notice of any cancellations to avoid this fee.
Your time is also valuable and we do our best to provide timely service and provide a 2 day courtesy appointment reminder call. We do understand that sometimes things happen beyond your control, please call our office to discuss any special considerations.
Medical Records
In compliance with HIPAA, patients are required to have a signed medical records release form before any records can be released. Phone consent is not enough. Most requests an be faxed out the same day. Please allow one week for processing.
When bringing records to the office for physician review, please bring copies. The doctor will review and scan only what he wants in your chart. Keep your originals, we will not mail you the unscanned records, they will be shredded.

Medication Prior Authorizations
If your pharmacy informs you that your prescription needs prior authorization please provide us with an alternate equivalent medication that is covered by your insurance whenever possible.
If you still require a prior authorization please do ALL of the following:

Have your pharmacy send the required paperwork to our office

Call our office directly at 247-7778, ext 4 option 1 and leave a message requesing a Prior Authorization including your name, the medication, and the pharmacy.
Once it has been submitted to your insurance company it may take up to two weeks for them to inform to your pharmacy of the determination.
